Specialized OCD Treatment Using ERP

Exposure and Response Prevention (ERP) is a first-line, evidence-based treatment for Obsessive-Compulsive Disorder, widely recommended by clinical bodies including the International OCD Foundation and the American Psychological Association.

ERP works by gradually and systematically exposing clients to the thoughts, situations, or sensations that trigger OCD, while preventing the compulsive response. Over time, the brain learns that anxiety naturally decreases on its own and that feared outcomes are far less likely or dangerous than OCD suggests.

At EBT Collaborative, ERP is delivered by clinicians who specialize in OCD treatment. We treat OCD across all subtypes and age groups using a structured, individualized approach.

OCD Treatment for Children in Northern Virginia

For children with OCD, ERP is adapted using age-appropriate language, games, and structured exercises that match the child's developmental stage. Parents are active participants in treatment from the start.

A key part of pediatric OCD treatment is reducing parental accommodation, the tendency of parents to participate in or enable compulsions to reduce their child's distress. Accommodation maintains OCD over time and is one of the most important clinical targets when treating children. Our clinicians coach parents on how to respond to OCD at home in ways that support recovery rather than reinforce avoidance.

For children who have co-occurring emotion dysregulation alongside OCD, we offer DBT-C which can be coordinated with ERP to address both presentations.

OCD Treatment for Teens in Northern Virginia

Adolescent ERP is delivered with family involvement tailored to the teen's age and developmental needs. Parents and caregivers are coached on accommodation reduction and on how to support their teen through the discomfort of exposure practice at home, where much of the important work happens between sessions.

For teens who have co-occurring OCD and significant emotion dysregulation, self-harm, or BPD features, our clinicians can coordinate ERP with our multi-family DBT program for adolescents. These two evidence-based treatments can be integrated into a coherent treatment plan rather than addressed separately.

OCD Treatment for Adults in Northern Virginia

Adult ERP follows a structured, graduated protocol tailored to the client's specific OCD presentation. Treatment begins with a comprehensive assessment that identifies the specific obsessions and compulsions, builds an individualized exposure hierarchy, and establishes clear treatment goals.

Sessions are active. Exposures happen in the room, not just as homework. Between sessions, clients practice assigned exposures in real-world settings, which is where the learning consolidates into everyday life.

For adults with a co-occurring trauma history alongside OCD, our clinicians can coordinate ERP with DBT Prolonged Exposure when clinically appropriate.

Individualized Intensive OCD Programming in Northern Virginia

For clients who need faster progress than once-weekly sessions allow, our individualized intensive programming provides concentrated ERP across multiple sessions per week.

Intensive ERP is appropriate for clients who:

  • Have not made adequate progress with standard weekly ERP
  • Need to make rapid progress within a defined period, such as before a school year or major life transition
  • Are traveling from outside the Northern Virginia area for a focused course of treatment
  • Have severe OCD that is significantly interfering with daily functioning

Research supports intensive ERP as an effective treatment format that can produce meaningful improvement over a shorter treatment period.

What to Expect at Your First Appointment

Your first appointment is a comprehensive assessment, not a treatment session. Your clinician will take a detailed history of your OCD presentation, identify the specific obsessions and compulsions involved, understand how OCD has affected your daily life, and assess for any co-occurring conditions that may shape the treatment plan.

From there, your clinician will collaborate with you to build an exposure hierarchy and explain exactly how the ERP process will work. Treatment proceeds at a pace you and your clinician agree on together. No one is pushed into exposures they are not prepared for.

Frequently Asked Questions

How long does OCD treatment take?

Most clients complete a standard ERP course in 12 to 20 weekly sessions. The timeline depends on the complexity and severity of the OCD presentation and how consistently the client practices exposures between sessions. Intensive formats can produce significant gains in a shorter period.

Is ERP uncomfortable?

ERP involves deliberately approaching what triggers anxiety, which is uncomfortable by design. However, the discomfort is structured, graduated, and collaborative. Clients are never pushed to jump to the most difficult exposures immediately. The pace builds gradually as skills and confidence develop.

Can OCD be cured?

OCD is a chronic condition for most people, but it is highly manageable. Many clients who complete a full course of ERP experience significant symptom reduction and are able to live full, functional lives with minimal interference from OCD. Ongoing maintenance exposures help prevent relapse.

Does my child need to want to do ERP?

Motivation matters but is not a prerequisite for starting treatment. For children and adolescents, parents often play a significant role in supporting engagement with ERP. Our clinicians are skilled at building motivation collaboratively with young clients and their families during the early stages of treatment.
